What this website collects

This website has no contact form and does not collect health information. It does not use Google Analytics, Microsoft Clarity, a Facebook pixel, advertising tags or cookies. If aggregate traffic counts are collected in future they will come from a cookieless service that does not identify individual visitors.

Where your health information lives

Appointment requests, intake paperwork, secure messages, telehealth and billing all happen inside the client portal, which is run by Practice Picnic under a Business Associate Agreement with Revival Therapy & Consulting, LLC. Nothing you enter in the portal passes through this website.

Email is fine for scheduling questions. Please do not send health details by email, because ordinary email is not a secure channel.

Notice of Privacy Practices

Revival Therapy's Notice of Privacy Practices, which explains how protected health information is used and your rights under HIPAA, is posted on this site, provided in your intake paperwork, and available on request by phone or email.
